How to Spot a Stale Training Matrix
A stale training matrix still opens, still looks complete, and still misleads. Spot it through mismatched names, dates that never move, missing files behind green cells, and more than one “master” copy in circulation — then fix ownership and status calculation before the next gate.

Pick ten people due on site this month and verify card faces against the sheet. If names, dates or files disagree — or if two versions disagree with each other — the matrix is stale. Replace manual colouring with date-based status and appoint a single live owner.
Staleness signals
- Filename includes “final_v7_USE_THIS”
- Leavers still showing as current
- No review date in the last 30 days despite active sites
- Certificates live only in inboxes
- Amber/green painted without matching expiry dates
- Agency rows blank until the morning of induction
Recovery checklist
- Declare one live system or file and archive the rest read-only.
- Reconcile the active workforce list with HR/payroll/agency orders.
- Re-enter dates only from evidence.
- Attach files for critical requirements.
- Run expired and due filters; publish a gap list with owners.
- Set a weekly exception review tied to the programme.
